Umm get a worn out text font. Try http://www.dafont.com/ for some free fonts.

Its not "Worn out text" its "grunged text" all you do is make the background you want then make a new layer write the text you want on that layer and pull out the eraser but instead of just regular circle change the brush to somthing choppy.. You can get good grunge brushes at www.angryblue.com
